Macon County Public Health
Diabetes Services
Macon County Public Health offers those suffering from Diabetes a Self-Management
Education Program. This program is for adults diagnosed with Type 2 diabetes and their family members or
friends.
Classes are offered several times a year. The program includes:
- Ten hours of education on healthy eating, being active, checking blood
sugar, taking medication, problem solving, healthy coping, and
reducing risk for complications.
- The cost is covered by many insurance providers. The programs is
offered on a sliding fee scale based on income for those without insurance.
- Includes individual meetings with a Registered Dietitian and group
classes.
How to enroll:
- Obtain diagnosis of diabetes
- Have your doctor complete referral form on the second page
of this document

- We will call you to schedule an appointment
